How to order a Birth Certificate

Request In Person:

• Visit our office Monday – Friday 8:00am to 4:30pm
• To expedite the process, you can print out and complete the Birth/Death Certificate Application. Bring the completed applicaton to the office, along with the appropriate fee and the office will process your request while you wait.
• Cost $22.00 cash, check, money order, Debit/Credit cards (a 2 ½ % convenience fee / minimum of $2.00 applies). 

Request By Mail:

• Print out and complete the Birth/Death Certificate Application (one application per person request).
• Acceptable payment methods are money orders or personal checks (for personal checks, please include your Driver’s License number).
• Make checks payable to : Auglaize County Health Department
• Requests are processed and sent by first-class mail the following business day.
• Send your requests along with $22.00/copy to:

Request By Phone With Credit Card:

• Contact Vital Statistics Department at (419)738-3410.
• Phone orders available during regular business hours and processed same day.
• Shipping options available: 1st Class Mail Included. Next Day at an additional cost.
• Cost $29.00 ($22.00 certificate fee plus $7.00 processing fee by Vital Chek)

Request Online Using Credit Card:

• Safe and secure site via Vital Chek enter Auglaize as birthplace.
• Our office receives the order within 2 hours (during regular business hours) and process the same day.
• Shipping options available: 1st Class Mail Included. Next Day at an additional cost.
• Cost $29.00 ($22.00 certificate fee plus $7.00 processing fee by Vital Chek)
• Go to Vital Chek and follow the instructions.
