Birth Certificates

Our office provides certified copies of Birth Certificates for any person who was born in the State of Ohio, from December 1908 to the present. Statewide Issuance gives us the ability to obtain your Birth Certificate without the inconvenience of traveling to your birth county or applying by mail.


All residents born in Ohio can easily obtain their Birth Certificates right here in Auglaize County.

How to get a Birth Certificate

Request In Person:


  • Visit our office Monday – Friday 8:00am to 4:30pm
  • To expedite the process, you can print out and complete the Birth Certificate Application. Bring the completed application to the office, along with the appropriate fee and the office will process your request while you wait.
  • Acceptable payment methods Cash, Check, Debt/Credit Cards

$25.00 certificate fee plus a fee for using a Credit/Debit Card

- 2.5% convenience fee / a minimum of $2.00 for cards

Request By Mail:


  • Print out and complete the Birth Certificate Application.
  • Acceptable payment methods are money orders or personal checks (for personal checks, please include your Driver’s License number).
  • Make checks payable to: Auglaize County Health Department
  • Requests are processed and sent by first-class mail the following business day.
