Eligibility:

· Be under 21 years old.

· Be an Ohio resident.

· Be under the care of an approved physician.

· Have a possible medical handicap (diagnostic program).

· Have an eligible medical handicap (treatment program).

· Be financially eligible (treatment program only).

 

Diagnostic Program

Purpose:

· Rule out a medically handicapping condition.

· Diagnose a condition.

· Help to develop a plan of treatment.

 

Services:

· Office visits to approved physician, as well as diagnostic tests that are needed.

· Public health nurse services.

· Up to 5 in-patient days if needed.

 

Treatment Program

Purpose:

· To provide payment for ongoing services related to handicapping condition for medically and financially eligible children.

 

Services (not a complete list):

· Office visits to approved physicians.

· Prescription medications.

· Therapies.

· Hearing aids

· Surgery.

· Medical equipment.

· Public health nursing services.
