Auglaize County Visiting Nurses Association

Serving Auglaize County since 1956

 

The goal of the Visiting Nurses Association is to help clients remain in their homes and maintain a high quality of life.  We accomplish this by providing home health services to those who still need supportive nursing services when; Medicare, Medicaid, PASSPORT or private insurance is not an option or when insurance benefits have been exhausted.

 

VNA Nursing

The Association provides education and maintenance nursing to help foster independent living with a private physician’s order.  The client’s physician  is provided and update every two months.

 

Home Health Aides

Home Health Aides provide personal care for clients to allow them to remain in their home.

 

Who’s Eligible?

Auglaize County residents who meet requirement on “Level of Need Survey”.

It is intended to evaluate the level of need for home health services, and addresses 10 areas of concern.
