Auglaize County Health Department

Environmental Division

The Environmental Division’s activities are performed to ensure healthy and safe living conditions for the residents of Auglaize County.  The authority to inspect and issue orders to correct unsanitary or unsafe conditions comes from the Ohio Revised Code.  Ohio Administrative Code provides specific criteria for all inspections.  Fees and permits are based upon the actual cost that the Auglaize County Health Department incurs while providing the specific service.  Below is the list of areas the Environmental Division is responsible for:

             Fundraisers, Festivals, and other Food Booth Information:

 

· Temporary Food Establishment Application:  This is an application for festivals, fund raisers, and other operations that wish to sell food for a short amount of time.

 

· Mobile Food Establishment Plan Review Packet:  This packet provides information on getting a new fair trailer or food sales unit approved for licensing.

· Restaurant / Retail Food Plan Review Packet:   This packet is for people who are opening a new permanent operation OR remodeling a current operation.

 

· Detailed Restaurant / Retail Food Plan Review Packet: This packet is intended as a secondary packet.  The Auglaize County Health Department may require this packet be filled out in addition to the general packet.  You may choose to use this packet over the general packet (found above).

 

 

Public Swimming Pools and Spas:

Weekly Pool and Spa Operation Report:  This form must be filled out daily by all licensed pools and spas.

 

Equipment Notification RequirementThis form should be used by licensed pools and spas that are replacing equipment with something other than “exact” replacement.

 

Application for Plan Review Form:  This form should be used when making substantial alterations to a licensed pool/spa or building a new one.
