Vital Statistics

This office houses Records of births and deaths in Auglaize County  from December 1908 to the present.

If the person was born or died in Auglaize County, Birth Certificates or Death Certificates can be purchased:

Payment:        Copies are $22.00 each.

 

Instructions for obtaining a certified copy of a vital statistic record through the mail:

1. Requests are processed and returned by first-class mail on the following business day.

2. Print out and complete either birth certificate application or death certificate application for appropriate record needed (Please fill out one application for each different person):

3. We accept money orders or personal checks for payment.  If sending a personal check, please put your Driver’s License # on the check.  Make the check out to: Auglaize County Health Department

4. Send your request along with the appropriate fee to the address on the website.

1. Visit the office:  Monday — Friday, from 8:00 AM - 4:00 PM.

2. To expedite the process, you can print out and complete the appropriate form below.  Bring the completed form to the office, along with the appropriate fee and we will process your request while you wait. 

3. Cash, check, and money orders are accepted.
