This office houses records of births and deaths in Auglaize County from December 1908 to present.
*CHANGE* to Ohio law – an individual can purchase a BIRTH certificate at any health department. You do not have to request the birth certificate from the county you were born. This does not apply to death certificates. You still must contact the county in which the descendent passed away.
INSTRUCTIONS FOR OBTAINING A BIRTH CERTIFICATE Individual must have been born in Ohio
REQUEST IN PERSON 1.) Visit the office: Monday – Friday, from 8:00 a.m. – 4:30 p.m. 2.) To expedite the process, you can print out and complete the birth certificate application. Bring the completed application to the office, along with the appropriate fee and the office will process your request while you wait. 3.) Cash, check, or money order is accepted.
REQUEST BY MAIL 1.) Print out and complete the birth certificate application (Please fill out one application for each different person). 2.) We accept money orders or personal checks for payment. If sending a personal check, please put your valid Driver’s License # on the check. Make the check payable to Auglaize County Health Department. 3.) Requests are processed and returned by first-class mail on the following business day. 4.) REQUEST BY PHONE USING A CREDIT CARD 1.) Phone orders can be placed by calling (419) 738-3410. 2.) These orders can be placed during regular business hours. 3.) Credit card is the only form of payment accepted by phone. 4.) Our staff will enter your information into Vitalchek through the internet. 5.) The ADDITIONAL FEE is assessed by Vitalchek.

Certificates are $22.00 each, by mail and in person. Certificates are $29.00 each, by internet or by phone. |
