This office houses Records of births and deaths in Auglaize County  from December 1908 to the present.

If the person was born or died in Auglaize County, Birth Certificates or Death Certificates can be purchased:

1. In the office from 8:00 AM— 4:00 PM M-F

                                            OR

Write the following information on a piece of paper for each  individual’s birth certificate:

· Full name at birth

· Date of Birth

· Place of Birth

· Father’s Full Name

· Mother’s First, Middle, and Maiden name

· Address of where birth certificates are to be sent.

· Daytime phone number in case we need to contact you.

· Payment

 

Death Certificate:

Write the following information on a piece of paper for each  individual’s death certificate:

· Full name of deceased person

· Date of Death

· Place of Death

· Address of where death certificates are to be sent.
